Terms and Conditions
Parent Portal Agreement
Effective Date: June 1, 2026
1. Acceptance of Terms
By creating a Parent Portal account and clicking "I Agree," you (the parent or legal guardian) acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions. If you do not agree, please do not create an account or enroll your student.
These Terms apply to all services offered by CREATE 308, including dance, music, and musical theatre programs, as well as pop-up workshops and special events.
2. Parent Portal Account
To enroll your student, you must create a Parent Portal account. You agree to:
Provide accurate and complete information during registration
Keep your login credentials secure and confidential
Promptly update your account if any information changes
Accept responsibility for all activity that occurs under your account
CREATE 308 reserves the right to suspend or terminate accounts that contain false information or violate these Terms.
3. Student Enrollment & Registration
Enrollment is complete only when: (a) registration has been submitted through the Parent Portal, and (b) applicable tuition or fees have been received. Spots are not guaranteed until both steps are complete.
CREATE 308 reserves the right to place students in appropriate class levels based on instructor assessment, regardless of prior experience or level requested.
4. Tuition, Fees & Payment
Tuition rates are published on the CREATE 308 website and subject to change with reasonable notice.
Tuition is due according to the payment schedule established at enrollment.
Returned payments or declined transactions may result in a service fee and temporary suspension of enrollment.
Drop-in classes and pop-up events require payment at the time of registration.
Tuition is non-refundable unless a class is cancelled by CREATE 308.
5. Attendance & Cancellations
Regular attendance is important for student progress. We ask that you notify CREATE 308 in advance of any absence. Missed classes due to student absence are not refunded or credited.
CREATE 308 may cancel classes due to instructor illness, inclement weather, or other circumstances. We will notify families as soon as possible. Cancelled classes will be made up or credited at our discretion.
6. Code of Conduct
CREATE 308 is committed to maintaining a safe, welcoming, and respectful environment for all students and families. By enrolling, you agree that your student and family members will:
Treat instructors, staff, and other students with respect
Follow all studio rules and instructor directions
Refrain from disruptive, disrespectful, or unsafe behavior
Arrive on time and prepared for class
CREATE 308 reserves the right to dismiss a student from the program without refund if conduct issues cannot be resolved.
7. Photo & Media Release
By enrolling your student, you grant CREATE 308 permission to photograph and/or video record your student during classes, rehearsals, and performances. These images and recordings may be used for studio marketing, social media, website content, and promotional materials.
If you do not wish to grant this permission, you must notify CREATE 308 in writing at info@create308.com. Opting out will not affect your student's enrollment or participation.
8. Health, Safety & Medical Authorization
You represent that your student is in good health and physically able to participate in the enrolled activities. You agree to inform CREATE 308 of any medical conditions, allergies, or physical limitations that may affect your student's participation.
In the event of a medical emergency, you authorize CREATE 308 staff to seek emergency medical care for your student if a parent or guardian cannot be reached. You agree to be responsible for any resulting medical costs.
9. Assumption of Risk & Liability Waiver
You acknowledge that participation in dance, theatre, and music activities carries inherent risks, including the possibility of physical injury. By enrolling your student, you voluntarily assume these risks on behalf of your student.
To the fullest extent permitted by law, you release and hold harmless CREATE 308, its owners, instructors, staff, and volunteers from any claims, damages, or injuries arising from your student's participation, except in cases of gross negligence or willful misconduct.
10. Privacy & Data Use
CREATE 308 collects personal information (such as names, contact details, and payment information) solely for the purpose of managing enrollment, communicating with families, and operating the Parent Portal. We do not sell or share personal information with third parties except as necessary to process payments or comply with legal obligations.

11. Changes to These Terms
CREATE 308 reserves the right to update these Terms and Conditions at any time. We will notify families of material changes via email or through the Parent Portal. Continued use of the Parent Portal following notification constitutes acceptance of the revised Terms.
12. Governing Law
These Terms and Conditions shall be governed by the laws of the State of Nebraska. Any disputes shall be resolved in Hall County, Nebraska.
